- Download the Policy Assignment Form.
2. Check Out the Policy
* Fill out the form with the following details:
* Current policyholder information (name, address, policy number) * New policyholder information (name and contact details) * Transfer effective date
- Ensure signatures from both the current and new policyholders are included.
