This procedure explains how agents can request a change to the Agent of Record (AOR) on an NFIP policy through the Equinox platform. A change in the AOR assigns a new agent to the policy while maintaining all other policy details intact.
The process requires a signed request from the policyholder.

Procedural Steps

1. Prepare the AOR Change Form

Download the AOR Change Form from the Equinox platform.

Fill out the form with the following details:

  • Policy number
  • New agent information
  • Policyholder details

2. Obtain Policyholder Authorization

Secure a signed policyholder authorization form granting permission for the AOR change.

Ensure the form includes:

  • Policy number
  • New agent details
  • Policyholder’s signature

3. Submit the AOR Change Request

In Equinox, open the "Change Requests" menu and select "Change Agent of Record." Enter the new agent’s information as provided in the authorization form.

4. Attach Supporting Documentation

Upload the signed policyholder authorization form and any other required documentation. Confirm that all uploaded files are properly labeled and linked to the policy record.

5. Review and Submit

Review the details entered for accuracy and completeness. Submit the AOR change request for processing.
